Service Repair Form:

If you would like to send your unit in for repair, please complete the following form: Service Repair Form.

1. Print out two copies of the attached service form, one for yourself and one to send with the unit.

2. Indicate the model and serial number of the unit.
3. On the service form, please describe the issue that you are having.
4. On the shipping label, please put ATTN TO: SERVICE DEPARTMENT.
